PHARMACOLOGY:

Pharmacodynamics:

Pirfenidone is a pyridine molecule with anti-inflammatory and antifibrotic activities both in vitro and in vivo. It inhibits the production and activity of TGF- beta that stimulates collagen synthesis and inhibits its degradation. It reduces other mediators of fibrogenesis like connective tissue growth factors it is also reduced the levels of platelet derived growth factors, A and B in a hamster model of bleomycin-induce lung fibrosis.

INDICATIONS:

Pirfenidone is indicated in the treatment of idiopathic pulmonary fibrosis.

DOSAGE AND ADMINISTRATION:

The dosage and administration is depended upon the medical condition of the patients. However, 200 mg thrice a day is recommended as the initial dose after a meal. Also you may also increase the dose if the potential benefit outweigh your medical condition and depending upon the symptoms. But, in the case of gastrointestinal symptoms or weight loss, then decrease the dose or discontinue taking the drug.

CONTRAINDICATION:

It is contraindicated in patients having known hypersensitivity to the product and its component used in the formulation of this very drug. This drug is highly contraindicated in patients having renal and hepatic dysfunction, women who are pregnant or plan to become pregnant or breast feeding.

WARNING AND PRECAUTIONS:

Pirfenidone causes an abnormal chromosomal structure on exposure to light in genotoxicity tests. Patients should be advised to protect themselves from the exposure to light. Therefore, it is recommended to wear long-sleeved clothing when outdoors or apply sunscreen. It may sometimes cause rash or itching, so if you face any of these, immediately consult your doctor. It is suggested to check your liver condition periodically if you are under the treatment of this drug because it may rise in the aspartate transaminase and alanine transaminase levels, suggestive of jaundice. Caution should be given when administrating on pediatric and geriatric patients.

RECOMMENDATIONS FOR LIVER FUNCTION MONITORING :

1) Prior to the medication, liver should be check out in all patients, then monthly for the first 6 months and every 3 months thereafter.

2) It is suggested not to perform any heavy tasks like driving machine or any hard work jobs during the treatment, because it may cause the feeling of drowsiness and dizziness.

DRUG INTERACTIONS:

The effectiveness of Pirfenidone is reduced if it is administered with fluvoxamine that inhibits CYP1A2 and several other CYP isoforms. Avoid smoking, alcohol and other drinks that can be interacted. Tell all your medical conditions and all your prescription and non-prescription, including herbal products to your doctor.

SIDE EFFECTS:

The most common adverse effects are photosensitivity, loss of appetite (anorexia), stomach discomfort and nausea and elevated gamma glutamyl transpeptidase levels and AST (SGOT) and ALT (SGPT) levels.

OVERDOSAGE:

Overdosing is highly prohibited. It may lead to dangerous situations. If you suspected of overdosing, then immediately consult your doctor or seek medical attention as soon as possible.

STORAGE:

It should be stored in room temperature far away from the reach of children and pets.
